Insurance
I am an out-of-network provider. In this case, your insurance plan may be able to reimburse you for a good amount of the cost of sessions. It is your responsibility to find out what your insurance plan’s out-of-network benefits are. I will provide statements that you can submit to your insurance for reimbursement.
Questions to ask your insurance provider about your out-of-network benefits
If you are seeking insurance reimbursement, please contact your insurance provider (call the number on the back of your card) to review your out-of-network mental health benefits. Here are questions you can ask to learn more about your coverage:
When you call the number on your card, ask:
1. Do I have out‑of‑network benefits for mental health services?
2. Do I need a referral or preauthorization for out‑of‑network mental health visits?
3. What is my out‑of‑network deductible? Is it separate from my in‑network deductible? How much has been met to date?
Is the amount applied to my deductible based on the provider's actual fee or your plan's approved/allowed amount?
4. After I meet the deductible, what percentage is reimbursed for out‑of‑network psychotherapy and testing?
Is reimbursement based on the provider's actual fee or an approved/customary amount?
If approved/customary, what are the approved amounts for the specific CPT codes below in my ZIP code?
Suggested CPT codes to ask about, depending on your situation: 90791 – Intake (55 min)
90834 – Psychotherapy (45 min)
90837 – Psychotherapy (1 hour)
5. Is there a limit on the number of covered sessions per year?
6. What is the most I'd pay out of pocket for out‑of‑network services in a year? Is that a separate limit from in‑network, or do they share one combined maximum?
Does it include mental health services?
What counts toward it? Does the difference between the provider's fee and the plan's approved amount count, or only the “approved amount” portion?
7. How do I submit claims/superbills?
Can I submit online? What documents are required? What is the typical processing time?
What is the deadline for submitting out‑of‑network claims?
Tip: Ask for and write down the representative's name, date, and a reference number for the call.
